Support at home after hospital service

The Support at Home after Hospital service is available to older people (aged 65+) who live on their own without the support of family living nearby and family and friends’ Carers in West Sussex and in need of some practical and emotional support after they are discharged from a stay in hospital. The service is provided by volunteers who will telephone and visit clients for up to 6 weeks. This service is free of charge as it is funded by West Sussex County Council.

The volunteers will provide practical support to those who are recuperating from their hospital stay by assisting with light housework, shopping, preparing a light snack, picking up prescriptions, and will help to build confidence by accompanying clients to follow-up GP and hospital appointments.

The service aims to support people to recover and become independent again with referrals on to other services to ensure that longer term support can be put in place if needed.
